Partnering for Success: Development and Implementation of an Interprofessional Community-Academic Navigation Program

Abstract
Sacred Heart University College of Nursing and Western Connecticut Healthcare Network developed an innovative community/academic partnership program known as iCAN (Interprofessional Community-Academic Navigation). The goal was to navigate services for some of the most vulnerable clients in the community. Challenges are inevitable within any partnership, but the team combated challenges and created a successful collaborative student program. Increased awareness into social determinants of health as well as improved client navigation to community services are among the powerful outcomes of this productive partnership.
